High Prolactin and Fertility: What Happens Next?

High prolactin and fertility are linked because excess prolactin can interfere with the hormones involved in ovulation. It may contribute to irregular or absent periods and difficulty conceiving. But one raised result does not establish its cause or prove that you have a pituitary tumour. Confirmation and a careful history come before a treatment decision.

How High Prolactin Can Affect Your Cycle

Prolactin is produced by the pituitary gland at the base of the brain. It normally rises during pregnancy and breastfeeding to support milk production. Outside that context, persistent elevation is called hyperprolactinemia.

The Endocrine Society’s patient information explains that excess prolactin can reduce the hormonal signals supporting ovarian function. Periods may become less regular or stop, and fertility can be affected. Some people notice milk discharge when they are not pregnant or breastfeeding; others do not.

Irregular periods have several possible causes. A prolactin result should be interpreted as one part of the assessment, rather than treated as the answer to every cycle change.

Why a Raised Prolactin Result May Need Confirmation

Blood-test report and magnifier: confirm the result and find the cause

Prolactin can rise temporarily in circumstances such as exercise, breast stimulation and the effects of some medicines. The 2024 ASRM opinion on evaluating absent periods advises confirming mildly elevated results before making the diagnosis.

Your clinician may request a repeat test and give preparation instructions. Follow those instructions rather than assuming that everyone needs the same fasting schedule or appointment time.

A further check may look for macroprolactin, a larger form that can complicate interpretation of the laboratory result. Thyroid function and pregnancy status may also need review. Bring the complete report, including units and the laboratory’s range, not just the highlighted value.

Bliss Fertility’s guide to hormonal fertility evaluation explains how individual tests fit into a wider workup.

What Might Be Causing the Elevation?

Possible explanations include pregnancy or breastfeeding, an underactive thyroid, certain medicines and problems involving the pituitary gland. A prolactinoma is a usually benign pituitary tumour that produces prolactin; it is one possible cause, not a conclusion drawn from a single blood test.

The NIDDK guide to prolactinoma explains how blood tests and imaging contribute to diagnosis. An MRI may be appropriate when the findings suggest a pituitary cause. Whether you need it depends on the clinical assessment.

Give your clinician a complete list of prescriptions, supplements and herbal products. Do not stop a medicine, especially treatment for mental health or another ongoing condition, to try to lower the result yourself.

Treatment Depends on the Cause and Your Goals

Some patients need treatment for an underlying condition; others need a carefully coordinated medication change or monitoring. For a confirmed prolactinoma, medicines called dopamine agonists are commonly used to lower prolactin and control the tumour. The prescriber decides the drug, dose and follow-up.

Treatment can help restore reproductive function, but it does not guarantee conception or exclude other fertility factors. If pregnancy is a goal, ask how ovulation will be assessed and what to do when a pregnancy test becomes positive. Do not change treatment without the agreed plan.

Our guide to ovulation induction discusses a related fertility treatment, but it is not automatically the next step after a high prolactin result.

Make the Consultation More Useful

Consultation checklist: cycle history, current medicines and previous blood tests

Bring your cycle dates, current medicines and previous blood tests. Ask:

• Does this result need repeating, and how should I prepare?

• Could a medicine, thyroid problem or macroprolactin explain it?

• What would make imaging necessary?

• What is the treatment goal, and who will review follow-up results?

Tell the clinician promptly about new headaches or changes in vision. A clear explanation of the cause is more useful than trying to reach an online “ideal” hormone number.
