Thyroid and Fertility: What Your Test Results Mean

Thyroid and fertility can be connected: an underactive or overactive thyroid may disrupt menstrual cycles and ovulation. But a slightly abnormal blood result does not automatically explain difficulty conceiving or mean that you need treatment. The useful next step is to understand the full report, your symptoms and the wider fertility assessment.
Can Thyroid Problems Affect Ovulation?

The thyroid is a small gland in the neck that helps regulate how the body uses energy. When it is underactive, symptoms may include tiredness, feeling unusually cold, dry skin and heavy or irregular periods. These symptoms can have other causes, so they are reasons to discuss testing rather than a diagnosis.
The NIDDK explanation of hypothyroidism includes fertility problems among its possible effects. Thyroid disease can be one part of the picture; it should not replace assessment of ovulation, the reproductive tract and the partner’s fertility.
What TSH and Free T4 Actually Tell You

TSH is a signal from the pituitary gland telling the thyroid to make hormone. Free T4 measures thyroid hormone available to the body. As the NIDDK guide to thyroid tests explains, an abnormal TSH result may need additional testing to establish what is happening.
Read the result with its units, laboratory reference range and any previous values. A screenshot showing only one number leaves out information your clinician needs. Pregnancy status and relevant medicines also matter when interpreting the report.
For example, a raised TSH with a normal free T4 is different from a pattern showing clearly reduced thyroid hormone. Ask which pattern applies to you before comparing your result with someone else’s.
Does Every Raised TSH Need Treatment?

No single treatment rule fits every mildly raised result. Subclinical hypothyroidism generally means TSH is above the applicable range while free T4 remains normal. This differs from overt hypothyroidism, which involves clearer thyroid hormone deficiency.
The 2024 ASRM guideline on subclinical hypothyroidism finds insufficient evidence that this mild pattern causes infertility. It also does not support routinely using levothyroxine for this diagnosis solely to improve pregnancy or live-birth outcomes. Professional recommendations can differ, particularly across pregnancy and clinical contexts.
This does not mean established thyroid disease should be ignored. It means the diagnosis, symptoms and treatment goal need to be clear. Do not begin thyroid medicine, change a prescribed dose or stop treatment because of an online threshold.
Planning Pregnancy When You Already Have Thyroid Disease
If you already take thyroid medicine, tell the clinician managing it that you are trying to conceive. Bring your current prescription and recent results to the fertility appointment so both teams can work from the same information.
The American Thyroid Association’s pregnancy guidance stresses that established hypothyroidism needs a plan before conception and prompt review when pregnancy is confirmed. Hormone needs may change during pregnancy; ask your prescriber for clear instructions rather than making an unplanned adjustment.
Bliss Fertility’s overview of hormonal fertility evaluation explains the broader assessment. If cycles are irregular, our guide to ovulation induction offers related background; treatment selection still depends on the cause.
Questions to Bring to Your Appointment

A useful consultation starts with a complete report and a short history of your cycles and symptoms. Ask:
• What diagnosis, if any, does this result support?
• Do I need repeat testing or another thyroid test?
• Would treatment address a confirmed thyroid problem, or is its fertility benefit uncertain?
• Who should review my medicines if I become pregnant?
The aim is a clear explanation and an individual plan, rather than treating a number in isolation.
